調(diào)查問卷一般可以看成是由三大部分組成的:卷首語(開場(chǎng)白)、正文和結(jié)尾。
1.卷首語
問卷的卷首語或開場(chǎng)白是致被調(diào)查的信或問候語。其內(nèi)容一般包括下列幾個(gè)方面:
(1)稱呼、問候。如“XX先生、女士:您好”。
(2)調(diào)查人員自我說明調(diào)查的主辦單位和個(gè)人的身份。
(3)簡要地說明調(diào)查的內(nèi)容、目的、填寫方法。
(4)說明作答的意義或重要性。
(5)說明所需要時(shí)間。
(6)保證作答對(duì)被調(diào)查者無負(fù)面作用,并替他保守秘密。
(7)表示真誠的感謝,或說明將贈(zèng)送小禮品。
信的語氣應(yīng)該是親切、誠懇而禮貌的,簡明扼要,切忌啰嗦。問卷的開頭是十分重要的。大量的實(shí)踐表明,幾乎所有拒接合作的人都是在開始接觸的前幾秒鐘內(nèi)就表示不愿參與的。如果潛在的調(diào)查對(duì)象在聽取介紹調(diào)查來意的一開始就愿意參與的話,那么絕大部分都會(huì)合作,而且一旦開始回答,就都會(huì)繼續(xù)并完成,除非在非常特殊的情況下才會(huì)中止。
2.正文
問卷的正文實(shí)際上也包含了三大部分。
第一部分包括向被調(diào)查者了解最一般的問題。這些問題應(yīng)該是適用于所有的被調(diào)查者,并能很快容易回答的問題。在這一部分不應(yīng)有任何難答的或敏感的問題,以免嚇壞被調(diào)查者。
第二部分是主要的內(nèi)容,包括涉及調(diào)查
酒店標(biāo)志設(shè)計(jì)主題的實(shí)質(zhì)和細(xì)節(jié)的大量題目。這一部分的結(jié)構(gòu)組織安排要符合邏輯性并對(duì)被調(diào)查者來說是有意義的。
第三部分一般包括兩部分的內(nèi)容,一是敏感性或復(fù)雜的問題,以及測(cè)量被調(diào)查者的態(tài)度或特性的問題;二是人口基本狀況、經(jīng)濟(jì)狀況等等。
3.結(jié)尾
問卷的結(jié)尾一般可以加上1~2道開方式題目,給被調(diào)查者一個(gè)自由發(fā)表意見的機(jī)會(huì)。然后,對(duì)被調(diào)查者的合作表示感謝。在問卷最后,一般應(yīng)附上一個(gè)“調(diào)查情況記錄”。這個(gè)記錄一般包括:
(1)調(diào)查人員(訪問員)姓名、編號(hào);
(2)受訪者的姓名、地址、電話號(hào)碼等
(3)問卷編號(hào)
(4)訪問時(shí)間
(5)其他,如設(shè)計(jì)分組等。
